Book an appointment
If you need to see your GP, then you can book an appointment using our online system (Anima). Our appointments are booked at 10-minute intervals.
How to book an appointment
Book an appointment online to see or speak to your GP or other healthcare professional by using our online triage system. Click the link below to use our online triage system. You'll be able to state the type of appointment you feel you need.
Types of appointments
At Concord Medical Centre, we offer different appointments to help you get the care you need as soon as possible.
Routine appointments
We aim to give you a routine appointment to see or speak to a GP or other Healthcare Professional within 2 weeks of receiving your online request.
Urgent appointments
If your appointment is considered urgent, we'll arrange for a GP or other Healthcare Professional to see or speak to you on the same day.
You can also visit 111 Online or call NHS 111 for general medical advice.
Medication reviews
If you have a repeat prescription, we may ask you to come in for a regular review. We will be in touch when you need to come in for a review.
GP home visit appointments
If you are acutely or chronically housebound then you can request a home visit appointment. We always try where possible to assess patients at the surgery where we have the right lighting, room set up and access to medical equipment to manage your problem.
To book a home visit use our online system or call 01454 616 767.
Video consultation
You can book a video consultation with your GP by using our online system. Your GP will then follow up by sending you a text or giving you a phone call to book a video consultation. If your GP feels that it would be best for you to come to the surgery, then they will arrange a face-to-face appointment with you.
If you need help with your appointment
Please tell us:
- If there's a specific GP, nurse or other health professional you would prefer to respond.
- If you would prefer to consult with the GP or nurse by phone, face-to-face, by video call or by text or email.
- If you need an interpreter.
- If you have any other access or communication needs.
