How to give patient feedback

You can give patient feedback by completing our online form.

You can also give feedback by contacting the surgery directly or by completing one of our complaint forms at the surgery.

Making a complaint

You can make a complaint by email, post or over the phone.

We will acknowledge your complaint within 3 working days to and reply within 35 working days.

NHS complaints advocates support people to complain about the treatment or care that they or a friend or family member have received from an NHS service.
